At Optimal Weight Loss Institute, we value your time — and ours. To ensure focused, personalized care and reserve your dedicated appointment, a $50 booking fee is required at the time of scheduling.
This fee allows our team to prepare in advance for your visit and secures your appointment time. The booking fee is non-refundable and is required to finalize all appointments.
Once your appointment is confirmed, all required intake paperwork will be sent electronically via email. These forms must be completed and submitted by 3:00 PM the business day prior to your appointment. A clear photo or scanned copy of your driver’s license must also be emailed to our team before your visit.
If you need to cancel or reschedule, we kindly require at least 48 hours’ notice. Appointments canceled without proper notice — or missed entirely — will result in forfeiture of the booking fee and an additional $49 no-show fee.
These policies allow us to honor every patient’s time and maintain the high standard of intentional, individualized care we are known for.
